In serious neuroses, the psychotic part of the personality is hidden under the neurosis, just as the part of the neurotic personality is masked by the psychosis in the psychotic patient.

I noticed the alternating emergence of a sense of vitality and of psychic death in the intersubjective space formed by the transference– countertransference relationship (Ogden, 1997). What therefore seemed to emerge again in the analytic space were the patient’s primitive, distressing experiences and the co-existence of healthy parts alongside with the autistic ones. I was unable to assign specific meanings to the material for a long time in the first phase, because of poor symbolic communication, the lack of dreams, and the presence of somatic symptoms.

On the unconscious level, such patients become very attached to the analyst, although they expect the analyst to be hostile and to be lacking in understanding. On the conscious level, they continue to idealize the analyst and regard her as being omnipotent and omniscient. The patient pays more attention to the analyst’s tone of voice or facial expressions rather than the content of her interpretation; this can be compared to the impression the newborn has of acquiring an ally, who will act as a support for his life.
